relationship building to promote capacity and competence to facilitate behavioural change among
priority patient populations.

This routable presentation will be a highly interactive and engaging session, prioritising two-way
exchange of information between attendees, presenters and facilitators.

Session outline and content
1. Presession survey (via QR code) – aiming to collect data on participants’ confidence to facilitate
behavioural change among priority populations and identify participants’ most significant
challenges in practice
2.
Research update – overview of 5As framework papers, systematic review findings, current
availability of tools and resources, provision of resources

Case study description

Small groups – work through case study, identify and problem solve workforce capacity and
competence challenges facilitating behavioural change in the context of the case
5.
Post-session survey (via QR code) – opportunity for attendees to further elaborate on building
competence and capacity to facilitating behavioural change in priority patient populations
Goals
• Attendees take away new ideas and resources that they take back to their practice to care for
priority patient populations.
• Inspire members to participate and share their experiences and knowledge through the shared
challenge of facilitating behavioural change among priority patient populations.
